Attorney General ’s office conditionally approved the sale of St. Elizabeth Care Center, a 52‑bed nonprofit skilled nursing facility in North Hollywood, to affiliates of The Ensign Group, Inc., the office said.
Ensign has operated St. Elizabeth since June 1, 2025 after receiving approval from the California Department of Public Health; the proposed transfer is part of a larger acquisition of Providence facilities and St. Elizabeth is the only California facility in the deal, the office said.
The approval includes a package of conditions intended to protect current residents and preserve access to Medicare and Medi‑Cal; the conditions run from seven to 10 years and are tailored to this transaction, the office said.
Key requirements include maintaining Medicare and Medi‑Cal participation and reserving at least 30% of skilled nursing beds for Medi‑Cal beneficiaries; keeping direct‑care staffing sufficient for resident acuity and state and federal requirements; and employing a full‑time licensed nursing home administrator for consistent oversight.
The conditions also require safe transfers and discharges and adequate living space; they limit rent increases, payments to affiliated entities and new debt that could threaten financial stability or reduce direct care, including requiring Attorney General approval for rent increases above 2.5%.
The attorney general’s office will appoint a monitor to oversee compliance; the office said the conditions are designed to protect residents, preserve access to care and provide meaningful oversight.
